When queuing for a dungeon, sometimes if you instantly queue, the dungeon matchmaking menu does not show up. This started happening for me in the most recent patch.
Also, if the matchmaking completes during a loading screen, it fails automatically as you can’t accept or see the menu during a load.
Have you been reporting these bugs with the in-game report bug feature? That may give the devs more information (logs from in game) that they can use to diagnose and hopefully patch with.
