I have mett quite a few that lost the mainquest while playing, usually if they were in a group and get destrakted. And then leveled up 10 or so levels not knowing main quest is missing. Maybe you could get a compass mark for the location of the last main questgiver you talked with, and have him repeat his instructions as a speach bubble untill you get to the next part?
I will be adding an additional tag (MSQ) to the title of all quests in the journal that are or pertain to the main story quest to try and help this problem since I have heard this from a few people also. I can also go back and make sure the descriptions are informative enough to get you on the right path even if its been days or weeks since you accepted the quest.
